The RN Field Nurse has HIPAA restricted access to certain client information, and is an hourly per-diem, non-exempt Direct Care staff member with no guaranteed minimum number of hours per week. 

 

Reporting Structure:

  • The RN Field Nurse reports directly to the Director of Nursing and/or Nurse Supervisor.

 

Qualifications:

  • The RN Field Nurse shall hold a current, valid license in good standing as a registered nurse issued by the New Jersey Board of Nursing.

  • Graduate of an accredited school of nursing required.

  • Must have excellent interpersonal skills, ability to work with children, the elderly, disabled and sick and work effectively with the demands of the job.

  • Excellent verbal and writing skills and computer proficiency as required by the position; read, write, speak and understand English as needed for the job.

  • Extensive assessment skills.

  • Must be able to pass the competency examination(s) with a passing grade of 80%.

  • When assigned, must be available at all times during the hours of operation of the Agency for on-call, where clinical nursing expertise is needed.

  • Must be able to pass a criminal background check(s), social security verification, identity verification, and undergo random illegal drug alcohol screening.

  • If driving a vehicle, must have a valid driver's license, clear driving record, and auto insurance.

  • Must have malpractice insurance in the amount of $1,000,000 minimally.

  • Must have current CPR certification.

  • Must maintain the 30 continuing education units (CEUs) per NJ Board of Nursing requirements to renew licensure.

  • As required by federal law, must be eligible to work in the United States and to complete the required employment eligibility verification document form within the prescribed time frame upon hire.

 

Physical Requirements:

This position routinely requires work in a variety of home and community settings and travel to and from clients’ homes or community, either by public transportation or car.  The RN Field Nurse must be able to meet the physical demands of field work in client’s homes and in the community, with or without reasonable accommodation, and in accordance with Uplift Nursing LLC’s Health Requirements policy.  While performing the duties of this job, the RN Field Nurse is regularly required to  partake in verbal communication and active listening.  The RN Field Nurse is required to stand, walk, reach, and use fingers and arms in the provision of performing client treatments and client care, and perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing. 

 

Duties and Responsibilities:

  • Performs direct nursing care, as assigned by the Director of Nursing/Nurse Supervisor and implements the nursing plan of care under a plan of treatment that is prescribed by a licensed Health Care Provider to clients receiving community-based skilled nursing services.

  • Keeps the Director of Nursing/Nurse Supervisor informed of any and all changes to the client’s status, nursing plan of care changes, recommendations, and related nursing activities performed in the home.

  • Accurately documents care provided in implementing the client’s nursing plan of care.

  • Assesses teaching needs of the client/family and provides instruction as needed.

  • Demonstrates the ability to communicate effectively with other members of the health care team and staff of the agency.  Effectively communicates and interacts with clients/caregivers and families in a supportive manner.

  • Consistently reports appropriate changes in the client’s condition to the physician/Health Care Provider, and immediately reports/documents changes to the Director of Nursing/Nurse Supervisor.

  • Demonstrates sound judgment and clinical knowledge in planning and decision making.

  • Demonstrates competence in performing all care, procedures, and treatments per the physician’s/Health Care Provider’s plan of treatment.

  • Adhere to agency infection control and safety procedures by performing and documenting procedures and interventions in the home according to the established care plan in a safe and competent manner.

  • Consistently assumes and follows through on the responsibility for assignment.

  • Demonstrates the ability to function effectively under stressful situations.

  • Maintains confidentiality of all clinical and personnel information in accordance with current laws and regulations.

  • Participates in the Agency’s Performance Quality Improvement and Risk Management Programs, when requested.

  • Available for on-call duty, when required, for situations when clinical expertise is required.

  • Works with the Director of Nursing to ensure adherence to Agency policies and the delivery of safe client services.

  • Adheres to all of Uplift Nursing LLC’s policies and procedures, and regulatory requirements.

  • Any other duties as assigned by the Director of Nursing, Nurse Supervisor, or Owner.
