OUR SERVICES

STEP 1

Call us to discuss your needs. We will do an intake and if your situation is a good fit for our services, we will provide a service agreement and schedule an initial nursing assessment.

STEP 2

We will send a nurse to the home for an assessment. The nurse will determine the level of care required and if Uplift Nursing can match your needs with the right staffing.  Note that an assessment is the final determination of whether or not we can proceed with care.

STEP 3

When we are able to proceed with a care plan, we will reach out to the primary care physician for an order for home care and we will begin scheduling nurses.

  • Do you create a personalized care plan for each client?
    Yes, creating personalized care plans is crucial to address each client's unique needs. Our approach emphasizes collaboration between our nurses and the client's doctor, ensuring a thorough assessment and nursing plan of care.
  • How do you communicate with clients and their family members regarding any issues, concerns, or recommendations?
    Any issues, concerns, or recommendations are addressed promptly through transparent and effective communication to ensure the best care for the clients. A supervisory nurse is available 24 hours 7 days a week. All communications are strictly between Uplift Nursing and the client, the client’s authorized representatives, and authorized healthcare providers.
  • Do you take insurance?
    All Uplift Nursing services are self-pay. While we do not accept insurance, we will assist clients who need to submit claims for long-tern care insurance.
  • Are you licensed and insured?
    Yes, Uplift Nursing is licensed and insured. The agency is registered with the NJ Office of the Attorney General Division of Consumer Affairs and is fully accredited for community based skilled nursing services with the Commission on Accreditation for Home Care.
  • I need help immediately. How soon can you have a nurse to my house?
    We strive to prioritize cases that may need prompt assistance. Upon completion of the necessary paperwork as well as an assessment, we can start providing care given available staff. We recognize the urgency some clients may have, and we will work with you to expedite the process and make every effort to meet your needs.
